The FAA has wisely developed several programs that encourage us to learn from our own small mistakes.   Most pilots are familiar with the NASA form and its role in the Aviation Safety Reporting System.  The Voluntary Disclosure Reporting Program is designed to promote safety in the aviation company environment rather than on an individual basis.  The Voluntary Disclosure Reporting Program is closely related to the Aviation Safety Action Program (ASAP) which encourages air carrier and repair station employees to voluntarily report safety information that may be critical to identifying potential precursors to accidents.
